Provide direct nursing care to patients in their homes in accordance with the physician's plan of care
Perform comprehensive patient assessments and develop individualized care plans
Administer medications, wound care, IV therapy, and other nursing procedures as required
Monitor patient progress and update care plans as necessary
Educate patients and families on disease management, medication use, and health maintenance
Coordinate care with physicians, other nurses, therapists, and home health aides
Document all patient care activities accurately and in a timely manner
Maintain compliance with federal, state, and local home health regulations

Qualifications:

Current and active RN license in CA
Graduate of an accredited nursing program
Minimum [1-2] years of nursing experience, preferably in home health or community health
Current CPR certification
Valid driver's license and reliable transportation
Strong organizational and time-management skills
Proficiency with EMR systems (e.g., OASIS documentation experience preferred)

Preferred Skills:

Experience with chronic disease management
Familiarity with home health regulations and standards
Bilingual skills a plus
