care and discharge planning in an acute hospital setting Assess
medical necessity per evidence-based guidelines Develop and update
patient care plans Manage disease and needs assessments, including
ordering DME Ensure compliance with CMS, CPT coding, Department of
Health, DRG, and HIPAA regulations 27901373EXPTEMP Job Requirements
Minimum 2 years acute medical care management experience in care
coordination and discharge planning Proficient in Epic charting
system Travel experience required; first-time travelers considered
