Current RN license required. Certifications: Relevant nursing
certifications preferred. Must-Have: - Experience with assessing
patients. - Experience with care coordination. - Experience with
discharge planning. - Knowledge of computerized documentation.
Description: This position requires a registered nurse to work as a
case manager. The nurse will be responsible for overseeing the care
coordination of patients. Daily census may include a caseload of up
to 25 patients. The role involves determining medical necessity
based on evidence-based guidelines. The nurse will engage in
discharge planning to ensure safe transitions for patients.
Knowledge of acute hospital admission criteria is essential.
Familiarity with CMS guidelines is required. The candidate should
have experience with CPT coding and billing. The position requires
adherence to HIPAA guidelines. Experience with disease management
and DRG systems is valuable. Interpersonal skills and the ability
to work collaboratively with multidisciplinary teams are necessary.
Candidates must have experience with health maintenance
organizations (HMOs) and independent practice associations (IPAs).
Weekend coverage is typically required once every four weeks,
depending on departmental needs. All travelers are expected to
float between facilities as needed. References will be required
from supervisory roles. The candidate should be willing to work
three of the five holidays each year.
