8 hours, days - Employment Type: Travel Day 5x8-Hour (08:00 -
16:30) Years of experience REQ: 2-year acute medical Care Manager
experience performing Care Coordination/Discharge Planning. Special
Procedures/Unit Details: • Experience with HMOs, IPAs, etc. •
Experience with Assessing patients, Care Coordination, and
Discharge Planning • Computerized documentation, required. • 2-year
acute medical Care Manager experience performing Care
Coordination/Discharge planning. Skills marked with * are preferred
