Conduct assessments to identify patient needs and develop care strategies. Achieve performance targets and contribute to team success. Collaborate with healthcare teams to ensure comprehensive patient support. Participate in professional development and training opportunities. Engage in continuous learning to enhance clinical skills and knowledge. Monitor patient progress and adjust care plans as necessary. Educate patients and families on care plans and health management. Participate in quality improvement initiatives to enhance patient care. Document patient interactions and maintain accurate records. Utilize data to track performance and improve service delivery. Provide support and guidance to junior staff and interns. Experience/Qualifications Valid RN license in California and relevant certifications. Commitment to ongoing professional development and learning. Experience in case management or similar patient care roles. Ability to develop and implement patient care plans effectively. Ability to work independently while contributing to team goals. Strong communication skills for patient and team interactions. Strong analytical skills to assess patient needs and outcomes. Proficiency in using electronic health records and data tracking tools. Experience in quality improvement initiatives in healthcare settings. Willingness to mentor junior staff and interns.
