Job Description:
We are seeking a Full-Time Clinical - Field Supervisor with
strong clinical knowledge and strong assessment skills in all
aspects of Home Health Care and/or Hospice to join our team. The RN
will be under supervision of the DPCS. The RN completes an initial
assessment, initiates the plan of care, and nursing procedures,
re-evaluates patient care, coordinates with the staffing
coordinator to ensure uninterrupted, continued care, and conducts
follow ups to ensure proper care.RN effectively and routinely
interacts with all disciplines, as well as clients, their family
members, doctors, referral sources, case managers, other team
members as well as third party individuals.Communicates any and all
information related to the patient with physicians, and staff.You
will also have a gas allowance and benefits.Job
Requirements:Qualifications/Requirements:* A minimum of one year's
experience* Has good communication and writing skills and reliable
transportation* Must have valid RN license* Must have valid
driver's license, clear record, and proof of car insurance* Willing
to see patients in the Los Angeles, San Fernando Valley, and Simi
Valley* Must have a current TB Test, chest x-ray, and physical exam
results* Have positive attitude towards work* Bilingual in
Spanish/English a plus* Can maintain professionalism at all
times
